Based on a fast Google search, it appears that a user mistake was most likely the cause of this pizza entirely collapsing in the oven. It seems that if you're going to bake the pizza on the rack, you have to put it in the oven completely frozen.

That way, the pizza has less moisture and you won't have to worry about it blowing up. It's okay to let it thaw a little, but you'll need to switch to a baking sheet and reduce the cooking time. Alternatively, you might order directly from Pizza Hut and avoid the middleman.
